FAQ

Do I need to activate my AXIS MasterCard® Debit Card?
No. Your card is automatically activated the first time you use it.

Is there a PIN for my AXIS MasterCard® Debit Card?
No. Choose the credit option when making a transaction with your AXIS MasterCard® Debit Card. Your card does not have a PIN.

Is there a list of eligible merchants/providers that accept the card?
No, but most healthcare providers/merchants that accept MasterCard® will accept the AXIS card.

Which pharmacies will accept my AXIS MasterCard® Debit Card?
Your AXIS MasterCard® debit card can be used at our member pharmacies to purchase prescription items only. Please click here or contact AXIS for the most recent list of member pharmacies. Funds in your FlexAccount can be used to reimburse you for qualifying purchases you make at non-member pharmacies, even though the card is not accepted. At non-member pharmacies and for over-the-counter (OTC) items, the Participant must pay for the prescription or OTC item and submit a claim to AXIS for reimbursement.

Can I purchase over-the-counter (OTC) items?
Beginning in 2011, many over-the-counter (OTC) items will require a doctor’s prescription in order to get reimbursed through a health reimbursement account such as the AXIS FlexAccount. Below is a detailed list of which OTC items will require a doctor’s prescription:

OTC Items must be purchased with out-of-pocket funds and submitted to AXIS for reimbursement. The AXIS Card cannot be used to purchase OTC items.

What if the provider/merchant doesn’t accept the AXIS MasterCard® Debit Card?
Participants can submit a Pay My Provider Request form along with a statement or invoice from provider listing: date of service, description of service and amount due. Once approved, the amount due to the provider will be deducted from the Participant’s FlexAccount and a check will be mailed on behalf of the Participant directly to the provider along with an Explanation of Payment (EOP). Participants can also pay for the service and submit an appropriate claim form, including documentation showing proof of payment, date of service and a description of the service.

What happens if the AXIS MasterCard® Debit Card is lost or stolen?
The Participant should contact AXIS Health Partners immediately by calling 1-888-417-AXIS (2947). AXIS will deactivate the lost/stolen card and issue a new one. The Participant’s Flex Account will be charged a $10.00 fee for replacement cards. If the AXIS MasterCard® Debit Card is used fraudulently, the Participant can be reimbursed for fraudulent use if unauthorized transactions are reported within 60 days of the date incurred.

How do I know how much is in my account?
By calling 1-888-417-2947, Participants can check their account balance or get answers to their account questions. Participants will need their 16-digit card number and four digit employer ID number (located on the back of your AXIS card) to obtain their balance using the automated system. The participant may also visit our website at www.axisflex.com to check their account balance.

How do I setup online access to my account?
You can access your AXIS FlexAccount online and view your balance and transaction history. To setup access to your account online, complete this simple registration process:

1. Go to axisflex.com and click on the Login link.
2. You will be directed to our Participant Portal page where you see a login screen.
3. Click on the Create Account link in blue below the Login button.
4. Enter your name.
5. Enter your Employee ID number (this is your social security number without dashes).
6. Skip the Employer ID section.
7. Enter your 16-digit card number.
8. Create a user ID and password for your account.
9. Complete the security questions and click Submit.
10. You now have access to your FlexAccount!

Why am I being asked to submit receipts for items/services I used my AXIS MasterCard® Debit Card to pay for?
The IRS requires that all Healthcare Card transactions must be verified by the Plan Administrator. We are required to verify purchases made with your AXIS MasterCard® debit card. For more information on the verification process, click here.
